Dear Space Coast residents:
Attached is a link to the Space Coast Health Foundation’s Community Health Needs Assessment (CHNA) survey link. We’re asking that you please take the survey and also pass along to interested parties in Brevard County for them to complete. The more responses we have to the survey, the richer the data we will have to analyze.
The CHNA, funded every three years by the SCHF, is an in depth look at our local residents’ overall health. Area agencies and organizations use the results of this anonymous survey in determining where best to direct resources and personnel to make the most effective health impact for Space Coast residents.
Your participation is extremely important so again, please consider taking the survey and sharing it with others.

VERY IMPORTANT WHEN SHARING THE LINK: Each respondent must take the survey by entering with the exact URL noted above. You cannot test the link then copy the URL from your browser’s address bar — it must be typed in directly exactly as it appears above. When the original URL is clicked, it initiates a unique browser session, assigns a session ID, and the URL in the address bar changes. Using this “session” link will result in an “expired” notice. Please share this warning with anyone who will be posting or sharing the link.
The online survey includes a couple of questions designed to ensure that respondents are providing truthful and thoughtful feedback. For example, you might notice the age question is duplicated or that there is an extra question designed to ensure respondents are paying attention; these are intentional and used for data quality checking.
